The following forms are for Cornell Engineering undergraduate and Master of Engineering (M.Eng.) students.
Some forms are part of electronic workflow, and some forms are not. For those that are not in electronic workflow (editable PDFs), students will need to either print and submit in person or send as an attachment via email to the appropriate office listed on the form, along with all required supporting documentation.
- Add or Drop a Double Major
- Application for College Credit Earned While in High School (PDF)
- For students who entered Cornell Fall 2023 and later.
- Application for Major Affiliation
- This form can be submitted during the semester a student is enrolled in the final courses needed to meet a major’s affiliation criteria. Please note, decisions will be communicated to students once grades are posted for those courses at the end of the semester.
- Change of Major
- Current Semester Petition
- Use this form for curriculum substitution and over 20 credit hour requests for the current enrolled semester. For courses enrolled in a previous semester, see the Past Semester Petition.
- Over 20 credit hour requests will not be reviewed during the pre-enroll period, only once the Add period for that semester begins.
- Not intended for Major-Approved Elective petitions. Affiliated students are advised to contact their major department for information regarding major-specific petition processes.
- High School Guidance Counselor Certification Form (PDF)
- For students who entered Cornell prior to Fall 2023.
- Liberal Studies Petition (PDF)

- Minor Certification Form (PDF)
- To be sent to the department offering the minor.
- Past Semester Petition
- Use this form to petition enrollment changes or curriculum substitution requests for a past semester.
- Not intended for Major-Approved Elective petitions. Affiliated students are advised to contact their major department for information regarding major-specific petition processes.
- Release of Information Form
- Use this form to request that the Engineering Registrar release information on your behalf or have a letter of verification sent to a third party.
- This form is not to be used to request an official transcript from the University. To request an official transcript, please go to Cornell University Registrar: Ordering Official Transcript.
- Time Conflict Permission Form
- Use this form for time conflict requests for the current enrolled semester.
- These forms will not be reviewed during the pre-enroll period, only once the Add period for that semester begins.
